Ladies when you are requesting are you making sure all the information needed is in the box. If not it will be denied and you need to resubmit.

I have filled it out entirely twice. I really am not sure what happened! I don't have a working computer right now (only my phone), so I can't try it again, but just wasn't sure if it was a fluke or if it has happened to others. Thanks!
