Refusal to Vaccinate Form

I delay my vaccinations, so my 4 year old is up to par. My 18 month old has never been vaccinated. Their pediatrician has never asked me to sign anything one way or the other. Not one thing. I'm not sure why you think I should be signing off on all of this information, but I'm willing to bet they are not legally compelled to have me sign anything.

Agreed the vast majority of docs don't inform of vaccine reactions beyond soreness at the injection site and maybe a low grade fever. I've never heard of/know of any doc requiring a parent who wants to vaccinate to sign a form stating they have been made aware of side effects and contraindications. I've only heard of it since joining parenting forums.
