I would say reflux too it's not necessarily a dairy issue though. Before trying that I would look at other options. Is there a seasoning or food you eat a lot? My youngest cant tolerate anything with tomatoes (pizza sauce, spaghetti sauce, salsa), taco seasoning, spicy barbecue, pepperoni, sausage (even breakfast sausage), chocolate, orange juice and other acidic fruit/juice and I'm sure there's more I'm not thinking of right now! So I eat a pretty bland diet but it is worth the trouble for a happier baby best of luck figuring it out! It is not a fun time!

Eta: some babies really don't like garlic or onion, I guess it can make the milk taste different!

My little girl did this right around 4 months too. I have a very heavy let down and I think it was partly this, coupled with the fact that I had been away from her for a couple hours over the course of several days. She eventually stopped after lots of snuggle time over about a week or so. Good luck!

I agree with reflux, both my girls have it and in their cases it is food related. We did elimination diet and found they bith have multiple food sensitivities. It was new to me too, none of my boys would act like that.

I was going to mention a heavy letdown too. My sisters 1st acted like that for the 1st couple months and she said she letdown was so strong if she didnt have something over the other breast it would shoot milk out. I hope you get it figured out soon mama

I was going to say forceful letdown...I would try gently removing him frim breast when you feel letdown coming. I would keep a burp cloth right next to my babies and just let the worst of the spray out into it. Mine eventually got used to it once they were a bit older.

Definitely sounds like reflux. I would talk to your pedi about it.

ETA: If you decide to give up dairy, you don't have to give up things cooked with dairy products in them. Just don't drink milk, eat yogurt...etc.

It's been years and years for me....But, can he breath through his nose? I mean, is your breast pressing against his nose? If your breast is blocking his airway, he would flail.

I know that sounds like a stupid thing to ask, I'm not trying to state the obvious, I'm just putting out other ideas.

I've been going through this with my little one too and it's oversupply and forceful letdown. For awhile I thought it was reflux. Try nursing him uphill. I latch him on then recline all the way. This made a huge difference!

That's not true. A true dairy elimination diet contains no dairy at all. You have to read labels closely and exclude foods with ingredients like whey and casein as well.
