I'm taking him in to his Primary care Dr today. He's still running a low grade fever, he's whiny and clingy, he's not sleeping at night (waking up screaming), he's just not acting right.

My Mood:
A puncture wound to the head can be pretty severe. I would be more worried about complications with that than the vaccination.

This. Especially at night if he rolls over and puts any pressure on it. I do think it's wise to take him in to have everything checked out though.

I took him in yesterday.. they did a CT scan of his head and that was negative, so that's good. They also ordered bloodwork.. Dr called this morning and his white blood cell count came back high, so they took a urine sample today for a culture, and he has an appt to follow up on Friday.. in the meantime we're waiting for the urine culture to come back. He's doing ok, but still fighting a low grade fever and isnt quite himself.

You don't need a tetanus shot if the wound bled. Tetanus cannot live in an oxygenated wound.

This.

My kids are fully vaxed and have never had fevers from any, including DTaP.

I would guess it's related to the wound itself. Two of my kids will get fevers from staying up too late, or from playing too rough, or falling down and getting a bad bruise. Their bodies just react with a fever when they are injured, even without infections.
