Does your kids doctor...

A have you sign consent forms before giving vaccines and B do you get a written record of what they got and when? I keep my own records for shots, and am so glad. Dd's doctor is claiming she missed a whole months worth and they don't believe me when I told them. When Dh told his sister who is a doctor freaked when she herd and said we should be getting a form at the end of visits with shots listed and we should be giving consent in writing. So do you sign before vaccines and get a record, or is your doc like mine?

I sign both for the shots we get and that we decline, and get a record. The record is recorded in the computer, I get a print out.

Yep, this. It's also all registered with the county health department.

I sign, but don't get a copy of record unless I ask

Oh gese, I had no idea, what do I do, besides changing doctors witch is going to happen, we have to find a replacement before dumping this one.

I sign, get asked if I want a printout, get an email of the visit, and have access to their online shot record/lab results.

My kid's pediatrician has moved to online record keeping so I don't get anything after visits. I can log in and see a summary of the visit 24 hours afterwards.

Whenever we make the next appointment, we are told what shots we will get the next time, and then in the rooms there are laminated charts that show what vaccines are given which visits, so we always check against the chart during the visit WITH the doctor.

I sign & get a summary of the visit when I check out. I also have access via patient portal, they'll print out any time I want, & it exists in the RHIO.

I don't sign if I decline and I don't sign when they are administered. I do get a paper summary of the visit.

No we don't sign. They give us a print out before each appointment about each vaccine the child will be given. I'm sure we would need to sign if we were declining them but we don't. They also remind us they are giving vaccines multiple times and then again before they actually go get the shots to give us time to decline or make any decision regarding them.
