Rash that won't go away, I need help!

My son is 19 months old, uncircumcised, and has a rash on his bottom. It started out with swelling/pain in his penis which he has had before when he was 13 months old. His pediatrician prescribed antibiotics and it went away. Now, he had the same thing and she prescribed a cream that reduced the swelling but it doesn't look the same as it did before. The cream is Mupirocin. He is supposed to put it on 2x a day for 30 days but I stopped a few days ago because he has broken out in a rash and I thought the cream caused it.

The rash looks like little scratches/scabs all over his bottom.

What is it and how do I cure it? I really don't want to take him back to his pediatrician. Thanks.

Did they do a culture to find out what you're dealing with? In some cases antibiotics can make things worse (cause yeast infections) so I'd take him back and make sure they do a culture. If it's on his penis, they only need to swab the very tip of the foreskin, they do not need to retract it.

Also, being intact has nothing to do with him getting rashes.

She didn't do a culture this time but did one last time. Although she never told me the results. I'm feeling like I may need to make another appointment. This makes me feel like such a bad mother. He didn't have problems until after he turned 1. He would break out a little and I changed detergents and was really careful with washing (still am) and all would be fine. Right now I am putting a little Balmex on him.
